Are you an older adult who wants information about how to continue living in your home for as long as possible?
Is an older family member having problems but unwilling to accept help?
Iona Consultation Services' team of knowledgeable and compassionate social workers and nurses helps older adults and their family members find solutions to these and other issues. Out Take Charge / Age Well Consultations address the specific need of independent older adults, and family members and others who are concerned about an older adult will benefit from our Family and Caregiver Consultations.

Take Charge / Age Well Consultations
Designed for older adults who are still living in their homes, Take Charge / Age Well Consultations provide customized information and referrals that can help you plan for your future. Commonly discussed issues include:
- How to remain in your home as you age
- How to decide whether to move or remain in your home
- Legal and financial concerns, such as powers of attorney and how to pay for services
- How to find the right medical help, including assessments for memory loss
- How to manage health issues, including memory loss
- How to find home care
- Which senior living communities might be the best fit for you
- Referrals to transportation services, support groups, and social activities

Family and Caregiver Consultations
- Are you looking for information and strategies to help you be an effective caregiver?
- Are you concerned that your aging parents need some assistance but don’t know how best to help them?
- Are you wondering what housing or home care options might be available for your older relatives?
- Are you and your parents and/or siblings disagreeing on next steps for your parents' care?
- Are you experiencing caregiver stress or burnout?
If you answered yes to any of these questions, turn to Iona Family and Caregiver Consultations. We specialize in helping older adults and their family members and caregivers cope effectively with the challenges that aging can pose. Commonly discussed issues include:
- Whether a loved one should move or remain at home
- How to find home care
- Which senior living facilities might be the best fit for your older relative
- How to manage health issues, including memory loss
- How to find the right medical help, including assessments for memory loss
- Legal and financial concerns, such as powers of attorney and how to pay for services
- Referrals to transportation services, caregiver support groups, and social activities
- How to communicate with a family member who is unwilling to accept help
- How to address concerns about a senior's memory loss, depression, substance abuse, or challenging behaviors

Who is eligible for a consult?
- Take Charge / Age Well Consultations: Persons age 50 and above who live anywhere in the metropolitan Washington, DC, area.
- Family and Caregiver Consultations: Anyone concerned about an older relative or friend, local or long-distance.
What are the charges for a consultation?
Consultations are at an hourly rate.
Suppose I need more in-depth service than a consultation can provide?
When more in-depth service is needed, Iona's team of social workers and nurse care managers can assess a senior's strengths and preferences, as well as challenges and risks. Creating a plan specifically tailored to each individual's situation, our care managers will identify resources, coordinate and monitor services that can enable the senior to continue living in their home, and, if needed, oversee a transition to another living arrangement. Your contribution to Iona helps them receive case management, support groups and participation in Iona’s award winning Adult Day Health, Wellness and Arts Center. Their loved one receives person-centered care during the day with exercise, art therapy, fellowship and stimulation while they receive a much needed respite from the responsibilities of caregiving.
