Care Management
Care management begins with a comprehensive in-home assessment to determine a senior’s preferences and needs. The assessment considers the senior's strengths and preferences, as well as the challenges and risks in the senior’s situation. Our care managers evaluate:
- What is important to the senior’s quality of life
- Physical health
- Memory and mood
- Ability to handle day-to-day needs
- Who is willing and able to help the senior
- Legal and financial issues
After completing the assessment, our team of social worker and nurse care managers work with seniors and family members to create a plan specifically tailored tot he senior's situation. The plan may include:
- Putting in place and monitoring services like home care that can enable the senior to continue living in their home
- Making sure they get to their medical appointments
- Monitoring medication compliance
- Keeping the senior's family and doctors informed about changes in their health
- Providing assistance with a transition to another living arrangement
Experienced care managers
Our care managers are experienced in dealing with the challenges of aging, including:
- Seniors who need assistance but are reluctant to accept help
- Memory loss, including Alzheimer's disease and other dementias
- Chronic illness
- Challenges with clutter, including hoarding
- Navigating benefits and subsidized services, as well as the complex healthcare system
- Sniors who have no support system
- Caregiver stress and burnout
Where is care management provided?
For a one- or two-visit in-home consultation and assessment, we can make home visits anywhere in the District of Columbia and in Montgomery County within the Beltway.
Ongoing care management is available to seniors living in the District in Wards 1 - 4, or in Montgomery County in zip codes 20814, 20815, 20816, 20910 and 20912.
What are the charges for care management?
Care management is offered on a sliding scale based on the senior’s income. Seniors living in Washington, D.C. in Ward 3 or parts of Wards 2 and 4 may be eligible for care management services subsidized by the DC Office on Aging.
