Consultation, Care Management and Counseling

Caregiver and Take Charge/Age Well Consultations

Caregiver and Take Charge/Age Well Consultations are provided by Iona’s experienced care managers (social workers and nurses) for caregivers and older adults.  They offer customized information that can help you plan for your future or that of an older relative. Commonly discussed issues include whether to move or remain at home, legal and financial concerns such as powers of attorney and how to pay for services, and finding home care and the right medical help. Care Management

Care management begins with a comprehensive in-home assessment to determine a senior’s preferences and needs.  Our care managers work with seniors and family members to create a plan specifically tailored to the senior’s situation.  The plan may involve putting in place services like home care or assistance with a transition to another living arrangement. Counseling Services
