Consultation, Care Management and Counseling
Caregiver and Take Charge/Age Well Consultations
Caregiver and Take Charge/Age Well Consultations are provided by Iona’s experienced care managers (social workers and nurses) for caregivers and older adults. They offer customized information that can help you plan for your future or that of an older relative. Commonly discussed issues include whether to move or remain at home, legal and financial concerns such as powers of attorney and how to pay for services, and finding home care and the right medical help. Care Management
Care management begins with a comprehensive in-home assessment to determine a senior’s preferences and needs. Our care managers work with seniors and family members to create a plan specifically tailored to the senior’s situation. The plan may involve putting in place services like home care or assistance with a transition to another living arrangement. Click here for more information.
Counseling Services
The aging process can bring emotional challenges. Iona’s counseling services are specially designed to address the psychological needs of older adults, such as anxiety, coping with illness, grief and the emotional pressures of growing older. That one hour of counseling is the difference between a life that continues to be lived in independence, or a life lived in unnecessary and expensive institutionalization.

Your contribution to Iona helps them receive case management, support groups and participation in Iona’s award winning Adult Day Health, Wellness and Arts Center. Their loved one receives person-centered care during the day with exercise, art therapy, fellowship and stimulation while they receive a much needed respite from the responsibilities of caregiving.
